The requisite demand draft must be purchased before submitting the online application form. The Demand Draft must be made in favor of "Chairman, JAM, IIT Bombay" on any Nationalized Bank, payable at Mumbai.

Enter all the details carefully in the application form. After you are satisfied that all the entries are correct, click on the SUBMIT button. On submission of the application, you will be allotted an application number.

A print version of the application form is generated in .pdf format (Acrobat Reader 8.0 or later version). The form will have the application number. Please note down the application number for any future correspondence with our office during the admission process of JAM 2012.

Press PRINT button for getting the print out of your application form. Take a black and white print out of the application form.

Take a good quality photograph of yourself and write your name and application number on the reverse. Then paste the photograph within the box provided on the printed application form. The photograph size must fit within the box (3cm x 4cm).

Put your signature within the space provided in the application form. Put your signature also below the Declaration after reading it carefully. An Application Form, without photograph and signature of the applicant, will be rejected.

The duly completed Online Application Form with your photo pasted on it and your two signatures along with the attested copies of all relevant certificates (SC / ST / PD / Affidavit for change of name, if applicable) and demand draft should be sent by speed post to :

Keep a photocopy of the completed application form, which you are submitting, with you for future reference.

An address slip will be generated as the second page of the application form. Cut along the dotted line and paste the address slip on the envelope containing your completed application form, and send to the address mentioned above.

The last date for receipt of completed Online Application Form along with the enclosures and Demand Draft
is 8^th November, 2011 (Tuesday) at the address stated above. This completes the ONLINE APPLICATION PROCEDURE.
